What this template is for
NPS shows who is ready to recommend the product and who is about to leave — and why. A 0–10 score plus one "what should we improve" question gives a number for the report and concrete reasons from unhappy users.
Why these defaults
- From the 5th visit: scores come from people who have used it, not newcomers.
- A card in the corner: the survey does not cover the work.
- Threshold 9: 9–10 are promoters and get the review link; 0–8 are asked what to improve.
- Once per visitor: asking again lowers the response rate.
